Adoption Application

The answers you give on this application will help us find the best possible match between you and the dogs available for adoption. Please make sure that all information submitted is current. If any information is omitted, the application will discarded.

We reserve the right to refuse any applicant and inspect the home environment. In addition, if we discover that any information on this application was knowingly falsified, we have the right to take back possession of the dog.

Please Note:If your application is approved, we will require a minimum donation of $300.00 to defray the costs of veterinary care for our rescued dogs. Also, at the time of adoption, you will be responsible for confirming that you have visited a vet and obtained heartworm preventative within the time allotted.
